Guesswork, but the small circling that holds the inner shaft in place may not have been following protocol.
This may have allowed the shaft to migrate outwards, spilling the oil and ripping the splines. The quaife splines look ok as best I can tell. I think they are made of proper metal and not Lotus cheese
There must be some metal bits somewhere but with most of the gearbox oil spewing out I hope that might have cleansed it.

I had a l/h driveshaft snap off in the diff just after I had the quaife fitted a few years ago …thankfully a new shaft was all that was needed…
As a precaution I think it’s prudent to use a new circlip each time a shaft is removed and refitted …
I assume you will be using a new shaft?

I could be wrong but I think Lotus stipulate a new circlip on refitment of the shaft , probably for the very reason we’ve both discovered to our cost !

Driveshafts were last put in by Essex Autosport and I don’t know if new clips were used.
